Making a Claim: identify your policy
The quickest way to access the claim forms you need is to identify the type of policy you have and then click on the relevant link - you'll be able to download the relevant claim forms and you'll find information on where to send them:
For United States policyholders

Group policies are taken out by companies who want to insure five or more people in their company. These policies will have the company's name written on the front of the policy document and have six or nine numbers, e.g. XY Company/ 123456 or XY Company/ 123456789 Click on the links below to access the relevant claim forms. Individual policies are usually taken out by individuals (although our Executive Income Protection product provides cover for businesses). Sick Pay Insurance policies will have the company's name written on the front of the policy document and will have seven numbers and one letter, e.g. Unum Limited/ B1234567 Click on the link below to access more information on making an online claim, or to download the relevant forms to fill in and post.
You can login to our member portal at any time to make claims and view your documents. Please ensure you include proof of payment for your treatment before submitting your claim. Unum Select policies will have the company's name written on the front of the policy document and will have seven numbers and one letter, e.g. Unum Limited/ A1234567 Note for Unum Select Employees/Claimants: You are a member of a Unum Select policy if you make payments that are automatically deducted from your pay and you joined the plan via a face-to- face or telephonic enrolment process at your place of work.
